What is Female Infertility?

Female infertility affects an estimated 48 million women worldwide. It’s defined as the inability to conceive after one year of regular, unprotected intercourse—or after six months for women over 35. Female infertility reasons can stem from a variety of issues within the reproductive system, including ovulation disorders, fallopian tube damage, and uterine abnormalities.

Causes of Female Infertility

Female infertility problems can result from a range of medical, structural, and lifestyle factors. Here’s a more detailed look at the key contributors:

1. Ovulation Disorders

 

  • Polycystic Ovary Syndrome (PCOS): A common hormonal disorder that affects how the ovaries function. PCOS is characterized by irregular menstrual cycles, excess male hormones (androgens), and multiple small cysts on the ovaries. These factors can disrupt ovulation, making conception difficult.
  • Hypothalamic Dysfunction: The hypothalamus regulates the release of hormones necessary for ovulation, such as luteinizing hormone (LH) and follicle-stimulating hormone (FSH). When this system is disrupted, often due to stress, weight changes, or excessive exercise, ovulation may be irregular or absent.
  • Premature Ovarian Insufficiency (POI): A condition where the ovaries stop functioning properly before the age of 40. This can lead to a significant reduction in the number of viable eggs available for fertilization, often caused by genetic factors, autoimmune diseases, or cancer treatments like chemotherapy.
  • Female infertility associated with anovulation: Irregular or absent ovulation is a leading cause of female infertility issues. Women experiencing anovulation may require hormonal therapies or fertility treatments to restore regular cycles.

2. Tubal or Pelvic Issues

  • Blocked or Damaged Fallopian Tubes: Fallopian tubes transport eggs from the ovaries to the uterus. Damage or blockages, often resulting from pelvic inflammatory disease (PID), sexually transmitted infections (like chlamydia or gonorrhea), or previous surgeries, can prevent fertilization or implantation.
  • Endometriosis: A condition where tissue similar to the uterine lining grows outside the uterus, often on the ovaries, fallopian tubes, or pelvic lining. This can cause inflammation, scarring, and adhesions, interfering with egg release, fertilization, and implantation.

3. Uterine or Cervical Conditions

  • Fibroids: Noncancerous growths that develop in or on the uterus. While often benign, fibroids can distort the shape of the uterus or block the fallopian tubes, impairing implantation or causing miscarriages.
  • Uterine Polyps or Scarring: Polyps are overgrowths of the endometrial lining, while scarring (Asherman’s syndrome) can result from uterine surgeries, infections, or conditions like endometriosis. Both can interfere with implantation or disrupt the uterine environment.

  • Excessive Alcohol Consumption: Can disrupt hormone levels and impair ovulation and implantation.

The primary sign of female infertility is the inability to conceive after trying for at least one year (or six months for women over 35). However, other potential signs of fertility issues include:

  • Menstrual cycles that are longer than 35 days, shorter than 21 days, irregular, or completely absent.
