What is Extended Culture?

Extended Culture helps us to identify which of your embryos are most suitable for transfer. It is a useful technique if you’ve produced good quality embryos in a previous treatment cycle, but not achieved a pregnancy.

This advanced IVF technique utilized to promote normal embryo development — ultimately increasing a patient’s chance of establishing a healthy pregnancy. This method is an effective supplement to IVF in certain circumstances, including patients who have had multiple failed IVF cycles or with previous poor embryo quality.

How Does it Work

A physician will collect a small sample of the patient’s endometrium immediately before the menstrual period and prior to the In Vitro Fertilization (IVF) cycle. Endometrial scratch biopsy is a transitory, outpatient procedure that does not require sedation. The collection of endometrial cells are treated, purified and frozen in an advanced embryology laboratory. Each cell is cautiously thawed prior to the egg retrieval.

After undergoing the In-vitro Fertilization (IVF) Cycle and embryo development has occurred, the cultured cells are placed in the petri dish with the embryo(s) where they will support embryonic development until the embryo transfer or cryopreservation takes place. intracytoplasmic Sperm Injection (ICSI) is performed by an embryologist, who surgically inserts one sperm into each egg.

Earlier, embryos were routinely transferred after a two or three days of culture, in the stage of 4 to 8 cells per embryo. However, this stage (cleavage stage)  can be reached by numerous potentially defective embryos and it is  impossible to distinguish a quality embryo at this early stage.

Development of new cultivation media has allowed for prolonged cultivation of the embryos in a laboratory. The extended culture allows for better assessment of the embryos, to select the embryo or embryos with adequate development for transfer.

The culture can be extended to 5-6 days (blastocyst stage) after oocyte collection. A blastocyst is an embryo that consists of more than 100 cells
In the uterus, the blastocyst starts to attach to the uterine lining in a process known as implantation. A higher implantation rate has been observed following transfer of blastocysts from the laboratory.

Advantages of Extended Culture

  • There is better synchronization between Endometrial preparation and embryo development.
  • With extended culture it is possible to choose embryos showing more promising development.
  • Less embryos to transfer: on day 2-3: 3 embryos, on day 5-6: 1 or 2 embryos to transfer.
